Procedure for Maturity Claim if Policy Lost |

I had a LIC Money Back policy which was done by my parents to save
income-tax 12 years back. As the agent was not known to me and lives far away
from my home, we were never in contact with that person after my father's
demise. But I kept paying the premium online in time . When I received the
letter from LIC on policy maturity, I found the main document required for the
whole procedure is the policy bond itself is missing.
I approached the branch with the maturity letter and I been
given 3 photo copied document, mostly which barely readable. The three
documents are as follows :-

Form 3815 B
- LIC
PARTICULARS AND CONSENT LETTER OF SURETY
- LIC
Query Form
Due to my busy schedule I contacted my tax consultant who
also does work as finance and investment consultant and handed over the three documents.
One of the documents have to be stamped with Rs.50 with all the matter
mentioned. But due to the poor quality of the copies those were not clearly
readable. So to be sure we tried to get the copies from nearby LIC offices. But we didn't
able to find the forms taking help of the two of my known senior agents including in
the LIC main branch(Hindustan Building,Kolkata).
Finally, we manage to get the fresh copies from
the very branch where I received the photo copies far away from my home. After
submission the rest of the process finished within a week or so.
But the main issue was to collect the right documents which
was not available in most of the branches I visited. So I thought I better
provide the documents for others facing similar problem in such situation.
